I'm just curious if anybody knows of the existence of a DVD database that lists what has gone out of print, and if there is one how accurate it might be?

The link to the DVD release spreadsheet in my Mobius Best of Film & DVD 2004 posting lists discontinued DVDs (use Excel's sort function), although some may just be UPC code changes. Still, it's a comprehensive resource if that's what you're after.

Every recent Criterion DVD I've bought has included a little insert listing all of their DVD releases from day one 'till the release era of the DVD in question, and lists which ones are OOP. So at the very least, Criterion keeps their customers up to date on which of their titles have gone OOP, which is a very valuable service to say the least.
